Freckles are small flat, brown marks that arise as a result of sun exposure. In adults they can be more persistent and, though mostly harmless, do have the potential to change.

Solar lentigos are small, flat, light to mid brown patches in sun-exposed areas.

In children, freckles can come after sun exposure and partly fade in the winter.

In adults, freckles are known as solar lentigos and can be more persistent.

Most often, they cause no bother or symptoms, but sometimes they can change.

Freckles and solar lentigos arise as a result of sun exposure.

They are particularly common in fair individuals though they can also be seen in darker skin types too.

Sun exposure results in more skin pigment (melanin) production by skin cells known as melanocytes.

Freckles or solar lentigos result from the accumulation of melanin.

If you are concerned about a potential change in a pre-existing freckle or lentigo, I advise you to ask a Consultant Dermatologist such as myself, to check them.

I start by taking a history of the onset of the pigmented patch, any changes in size, shape or colour and any symptoms.

I take other information such as your occupation, hobbies and general sunlight exposure, which may be relevant.

I will examine the changing lesion in more detail with the aid of a particular lens known as dermatoscope.

I also tend to offer a full skin examination during the appointment.

Most freckles turn out to be benign, but if I have any concerns, then I will offer you a diagnostic biopsy. I can often perform the biopsy on the same day as your appointment.

Treatment consists of an initial diagnostic biopsy. If the lesion is small enough, I completely remove the lesion and send it to the lab for analysis.

If there are any concerning changes, then further treatment will consist of complete removal of the suspicious lesion with a margin of healthy skin surrounding it.

You will usually need no further treatment beyond that stage.

Usually, I will determine your freckles to be harmless. Therefore, they do not require any specific treatment. It is essential that you take adequate sun protection measures to prevent any further sun damage or freckles from appearing. Additionally, using a good sunblock can help protect your existing freckles and allow them to fade partly.

I’m Dr Penelope Pratsou, a skilled independent Consultant Dermatologist based in Berkshire. I have specialist expertise in the diagnosis and management of all skin cancers, and in performing mole checks. I’m a trained skin surgeon and remove skin cancers, moles, skin tags, cysts and warts.

I also have invaluable experience in dealing with all skin conditions, from the common skin complaints of acne, rosacea, eczema and psoriasis, to the rarer and more complex skin problems, having seen it all through years of NHS work.

After I obtained my Membership to the Royal College of Physicians, I undertook rigorous specialist training in dermatology, before being appointed as a Consultant Dermatologist at the Royal Berkshire Hospital, Reading. There, I helped set up and lead a busy clinic for the diagnosis and treatment of suspected skin cancer. I was also actively involved in supervising and training both dermatology and GP trainees.

Alongside my increasingly busy private practice, I have maintained an NHS practice in Oxford in order to continue to engage with challenging cases and to develop my specialist interest in skin allergy.
